Appointment Check in information

Every time you bring in your pet, the staff will ask you the same set of questions.  This information is important to the doctor, and can change from visit to visit -- even if you were just here yesterday.  It may help you to think about the answers in advance -- especially if someone else is transporting your pet to the doctors visit. These questions include: how you pet's general attitude has been, how bowel movements and urination are, how appetite has been, what types of food (including treats, table scraps, etc) your pet is eating, and if they are on any flea or heartworm prevention, as well as any vitamins or supplements.
